Prescription drug coverage

HPA (Health Plan Administrators) offers a three tiered prescription drug coverage program that has copays of $10, $20, $50, brand name drugs with special pricing. Prices and copays will vary depending on class and dosage of medications. Prices for this plan begin at $19.99 per month for an individual and $34.99 per month for a family. Special pricing fo members over age 65.

Eligibility:
You and your spouse, no age limit, and you and your spouse's unmarried dependent children under age 19.
Children age 19 and over should apply separately. Child(ren) only under 19 (no adults will be using the card), listĘthe child's name and information as the applicant and the parent or legal guardian must sign the enrollment form. If there is more than one child to be covered, list the oldest as the applicant and the others as children. (select member + children).

Effective Date Information
If payment by credit card; the plan can be effective as early as 12:01 a.m. two days after the transmission date.
If payment by Check or Auto Check Withdrawal; the plan can become effective as early as 12:01 a.m. the day
after payment is received. However, the applicant can choose a later effective date not to exceed 60 days from
transmission date.
