LifeWise Passport™

In addition to providing benefit-rich coverage, LifeWise of Arizona Passport plans allow you to tailor your out-of-pocket expenses by providing five options for your deductible and three options for your office visit copay.

LifeWise Essentials™ 20

LifeWise Essentials 20 plans are ideal for individuals who want low premiums, coverage for major medical services such as urgent care, emergency and diagnostic services, and who are looking for upfront benefits such as preventative care, preventive immunizations and health education.

LifeWise HSA™ 20

LifeWise of Arizona offers three different HSA 20 plans each with a different deductible ($1,250, $1,700, or $2,500 for an individual) and coinsurance maximum. All have 20% coinsurance for in-network providers.

LifeWise HSA™

Our new LifeWise HSA plan in Arizona has a higher deductible ($5,000 for an individual) but no coinsurance. With the lowest premium of any of LifeWise’s HSA plans, you can divert the premium savings toward your health savings account.
